How to access the services you may need

PMA is available to you and your family seven days a week throughout the year in our two offices and after hours for urgent care. Please see below for the types of pediatric services we offer and the office guidelines for determining how to best use them. 

For your visit to PMA

Patient Registration: Please try to arrive five minutes prior to your appointment time in order to register with the Receptionist. Patients who are more than 15 minutes late may be asked to reschedule their pediatric associates appointments. Our office will ask for updated contact and insurance information at each visit. Please bring the correct forms with you to your appointment. Click here to see the forms you will need for your child.  You will be asked to verify that your insurance information is current and correct. You will be financially responsible for services not covered by insurance.

A valid insurance card must be presented at each visit. Pediatric patients are responsible for contacting their insurance company to confirm availability of coverage for services rendered prior to the visit. Services not covered by insurance are the financial responsibility of the patient/guarantor. With the great variety of new insurance plans, the ongoing changes to everyone’s coverage, and our limited access to your policy details we cannot be responsible for confirming your insurance benefits and limitations, so please check with your insurer before scheduling a visit. If you do not have coverage at the time of your child’s visit, please make arrangements for payment at the time of registration.

Copayments are due at the time of visit. Failure to pay the Copayment at the time of your visit will result in an additional $5.00 billing charge.
